This plan is available in three options:

  • 1-Year Option - One plan holder and 2 adults and 4 children as extensions

  • 2-Year Option - One plan holder and 2 adults and 4 children as extensions

  • 3-Year Option - One plan holder and 2 adults and 4 children as extensions

Create and access your own Electronic Medical Record (EMR) for free. The EMR is equipped with access to the EHMD Medical Imaging Viewer. The medical imaging module allows plan holders to convert paper-based records to web-based images.
